DOCUMENTATION FOR ADMISSION

Approximately three months elapse after a match and the start of orientation. During that time interns need to act swiftly and submit the required documentation.

NOTE: Without all the documentation you may not start the orientation

  • Complete a 5-panel drug screen.
  • Recent physical exam (within one year of orientation date), must state intern is in good health and can complete an internship. If a ‘reasonable accommodation’ is needed e.g. can’t stand longer than 15 minutes, it must be stated.
  • COVID-19 and Vaccinations and immunizations as recommended by the Centers for Disease Control and Prevention. Check with each facility to see what is required. CDC reference *Required immunizations are MMR, Tetanus, Hepatitis B, Chicken Pox vaccine or documentation of chicken pox e.g. letter from medical office.
  • NOTE: Many rotation facilities require a Flu shot and Hepatitis vaccinations. Proof of vaccinations with results should be provided.
  • Tuberculosis test must be negative and must have been completed within the last year.
  • If you drive, a legal state driver’s license and auto insurance must be provided.
  • Proof of medical insurance.
  • Original Verification Statement from an accredited didactic program in dietetics – sent after you graduate.
  • Official transcript with all your coursework wherever you attended school. Please send official transcripts if you attended more than one college or university.
  • Practice liability insurance – intern rate $20
  • If enrolled in a graduate program, acceptance letter on official letterhead. If graduated with a master's degree, official transcript or copy indicating date and degree conferred. To monitor the academic progress of intern’s currently enrolled in a master’s degree interns are required to sign an Authorization to Release Student Information.
